SASH (Support And Services at Home) Program is designed to provide personalized coordinated care to help adult participants stay safely at home. SASH helps Vermont’s most vulnerable citizens, seniors and individuals with special needs, access the care and support they need to stay healthy while living comfortably and safely at home. SASH is available in many communities throughout Vermont and serves primarily persons 65 and older and persons with disabilities. SASH communities include a care coordinator and wellness nurse who work in partnership with a team of community providers to assist SASH participants.
Created by Cathedral Square in 2011 and now operating in more than 100 Vermont communities, SASH supports participating seniors who live in affordable housing, as well as those receiving Medicare and living in communities where SASH is in place. The program is voluntary, with services provided free of charge to those who sign up. SASH has been documented to both improve health outcomes and reduce Medicare costs. The SASH model is currently being being demonstrated and replicated in several states.
